First Visit

During your initial visit, we will reserve one hour to take the time to learn about your individual needs and concerns.  Before you leave, we will present you with a treatment plan, explain your options for care, answer all of your questions, and schedule treatment. Please call Waushara Dental Associates to reserve your preferred date and time.

Please assist us by providing the following information at the time of your first visit:

• Any x-rays taken by a previous dentist, if applicable.
• A list of medications you are presently taking.
• Prior to your visit, please complete the new patient forms. If that is not possible, we also have copies at our office. Please arrive 15-20 minutes early to allow enough time to complete your paperwork.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert our office if you have a medical condition (artificial heart valve, joint replacement, history of infective endocarditis, heart transplant, certain congenital heart conditions) that may require antibiotic pre-medication prior to your visit.

X-Rays

During your comprehensive exam, a complete set of digital x-rays will be taken so we have a comprehensive view of your teeth and supporting structures for accurate diagnoses.
